Senior Care Medicare Program
In July of 1997, we launched a new health care plan called Senior Care. This new
plan is a Medicare Supplement and is underwritten by the United American
Insurance Company (UA) and Activa HealthGroup, Inc.. United American has a
strong tradition of service and financial stability to its customers.

Questions & Answers
Q. What Medicare Plans are offered?
A. If you live anywhere in the United States, and are 65 or turning 65,
we offer a Medicare Supplemental Plan F with the Activa Rx Card.
Q. Will I have to change my doctors or choose doctors from a provider
list? Will I have to give up my Medicare like the HMO’s plans?
A. No. If you are presently on a Medicare Supplemental Plan, you will see
no changes in your health insurance coverage. If, however, you are with an HMO
Plan and convert to the NEALR, Inc. Senior Care Plan you have complete
Q. If I have a pre-existing condition, will I have a waiting period?
A. No. Because we have a group plan, there are no penalties imposed for
pre-existing conditions. Your Senior Care coverage starts on your requested
start date.
Q. Will I have to file my Medicare claims?
A. No. When you receive a Medicare covered service, Medicare
automatically notifies United American. You no longer have to summit the
Medicare statement and your health care provider’s bill to get your claims paid.
Within 45 days of your start date, your Automatic Claims Filing Source will be
activated and everything will be done electronically.
Q. Will my doctor(s) accept this United American Insurance Company?
A. Yes. As long as your doctor(s) participate with Medicare, they must by
Federal Law, accept the supplemental provider.
Q. Who can I call when I have any questions or need Customer Service?
A. When you have questions or need Customer Service, you will call NEALR,
Inc.. We have access to insurance company account specialists that take care of
our members. You will talk to John A. Pernorio, he will personally assist you.
Q. I’m enrolled with an HMO. Do I have to inform them?
A. YES. You must write to the HMO and request them to terminate your
enrollment as of your requested start date with the NEALR, Inc. Senior Care
Supplemental Insurance Plan. It is very important that this is done.
Q. Who is eligible to participate in this Medicare Supplemental Plan?
A. Any labor retiree, widow of a labor retiree. Or parents of an active
union member age 65, or turning 65 before the proposed start date.
Q. How does the prescription part of this plan work? How many pharmacies
participate?
A. ActivaRx Card:
Copayments at participating pharmacy: 30 day supply or 90 unit doses maximum.
All name brand drugs will be priced at AWP (average wholesale price) minus 20%,
or, usual & customary, the lessor of the forgoing. All Generic Drugs: $10.00,
or, Usual & Customary, the lessor of the forgoing. If a Generic drug is
available, it must be taken. If no Generic drug is available, the above pricing
will apply. There are 46,000 participating pharmacies including most major
chains. This plan also includes an excellent Mail Pharmacy that will be
explained with your enrollment packet. There is no yearly cap, or deductible.
Q. What is the difference between BRAND-NAME and GENERIC DRUGS?
A. A generic medication is basically a copy of a brand-name medication.
The color or shape may be different, but the active ingredients must be the same
for both. Generic medications must meet the same quality standards as brand-name
medications. The FDA sets these standards and reviews all medications before
they are marketed.
Q. If I move to another state, can I keep this Senior Care Medicare Plan?
A. Yes. Our plan has portability. Once you are established as a resident in New
England, if you should move to another state, you can retain the Senior Care
Plan at the same premium you are paying here.
Q. Will my premium for the Senior Care Plan ever increase?
A. Yes. We have no control over increases. United American files rates
with each state and what ever is approved by the state will be the increase.
However, we try very hard to keep increases as low as possible.
